“Thrive Together,” a New Community Resource Guide

June 12, 2026 – Cook County is proud to announce the launch of Thrive Together, a community resource website designed to serve as a single, easy‑to‑use hub for local services, supports, and information. The new website, available at thriveccmn.org, connects residents with a wide range of resources to help them live healthier, more informed, and more connected lives.

Thrive Together was created to address a long‑standing challenge in Cook County: while many services exist, community members often don’t know where to find them. Following the COVID‑19 pandemic and an external Health Care Assessment, several local agencies identified a need for a comprehensive, centralized guide to community resources. Each agency maintained its own website and outreach efforts, but no unified “one‑stop shop” existed to help residents quickly locate the support they need.

This new website is the result of a collaborative effort among:

• Care Partners of Cook County

• Cook County Public Health and Human Services (PHHS)

• Grand Portage Health Services

• North Shore Health

• Sawtooth Mountain Clinic

The site is now live and available to the public at thriveccmn.org. Community members are encouraged not only to explore the guide and share it with neighbors, clients, and partner organizations, but also to help shape its continued development. Thrive Together is designed to grow and evolve based on community needs and is actively maintained by Cook County Public Health and Human Services staff. At the bottom of every page there is a feedback tab. If you notice a missing or inaccurate listing, or have suggestions for improvement, please share them. Your input ensures the site remains a true reflection of the resources our community needs and values.
