REQUEST FOR PROPOSALS FOR THE PROVISION OF HEALTH AND SOCIALS ERVICES FOR LAKE COUNTY RESIDENTS

Lake County Health and Human Services is currently requesting proposals to pro­vide health and human services programs for the period January 1, 2025, through December 31, 2025.

Proposals to provide services for Health and Human Services programs including mental health, developmental disabilities, employment and training services for the MFIP and DWP programs, and chemical dependency are being accepted. Propos­als for services must reflect mandates found in M.S. 245.61 (the only exceptions are specialized projects). Proposals may be submitted for the whole range of men­tal health services or any specific service area, and shall include special services to be provided in each of the mandated ser­vice areas.

All proposals shall include detailed de­scriptions of the service(s) to be provided, the qualifications of the provider(s), the annual budget of the provider, estimates of the number of people to be served and estimated units of service to be provided.

Proposals will be reviewed by Lake Coun­ty Health and Human Services staff. The Lake County Board of Commissioners will make the final determination of which pro­posals will be accepted.

Proposals must be received by the Lake County Health and Human Services De­partment by Friday, July 19, 2024, at 4:00 p.m.

NOTE: This request for proposals does not obligate Lake County to award mon­eys for mental health or other health and human services programs, and the Board reserves the right to cancel the solicitation if it is considered to be in its best interest.

The proposal must be signed by an au­thorized person and include the business name and address. The stated terms of the proposal must be valid for the length of the project.
